During COVID-19, Dr. Furman decided to change a few systems around to make the check-in process more efficient, safe and stream-lined.
Instead of laminated sheets with dry-erase pens, you can now conveniently access the forms on our website under the “Patient Forms” Tab! We’re all about saving trees and making the process of coming in seamless and easy so that we can do the important part– dentistry. The goal is also to eliminate time in the waiting room to maintain infection control and contact.
Particularly important is the COVID-19 Pre-Screening Form that is now required before every visit to ensure patient and team safety!
